1. I hereby agree that Promoter and those acting under the Promoter's authority may use my name, picture/portrait likeness and/or voice, for advertising and promotional purposes in connection with the EMC Unified Storage Guarantee Sweepstakes.

2. I hereby release and hold harmless Promoter, its affiliates, subsidiaries, advertising and promotion agencies, and Alterseekers, Inc., from any liability whatsoever, including death, and damage or loss of property, due in whole or in part directly or indirectly, from the acceptance, possession, use or misuse of the prize or participation related to the EMC Unified Storage Guarantee Sweepstakes.

3. I understand that if I am a U.S. resident, I will be responsible for all Federal, State and other taxes on the prize I may win. Therefore my social security number is necessary to provide for tax purposes.

4. I acknowledge that my entry may automatically be disqualified if this certificate is not received within 2 weeks of notification.
